    • Q: Which forms do I fill out?

     

    • A:
    • The OCF 23 (Minor Injury) is the most commonly used treatment form.  Every automobile accident automatically qualifies for the Minor Injury guideline amount of $3500 in total treatment services.  The adjuster determines whether your patient’s injury falls into the minor injury guidelines.  If you don’t know which form to fill out, always start with this one.
    • You complete the OCF23 to get approval for the plan, then the OCF21B to submit invoices for treatment
    • For major injuries, complete the OCF18 for approval and the OCF21C to submit invoices
