Receipt Requirements for Insurance Companies

 

In order for patients to claim their acupuncture treatments for reimbursement from their Insurance Companies, ALL receipts need to include the following:

 

A clinic/company stamp that includes business name, full address and phone number, and CMAAC registration number

The patient’s name, date of service, and an itemized list of each service provided, including a description of the service, and the cost of the service

 

Handwritten receipts are accepted, but must include one service only. For example, you may write up one handwritten receipt for acupuncture treatment, and a separate receipt for herbs. Handwritten receipts must include the criteria above.  Duplicate receipts must indicate that it is in fact a duplicate receipt by way of a stamp or writing on the receipt indicating that the duplicate is an exact copy of the original receipt.  Each client should have their own receipt. For example, if you are treating more than one member in a family, each family member should have their own receipt for treatment.  Invoices are accepted if they include a stamp that says PAID, and must include your business name and all business and client information listed above.

為了保證病人能夠順利地向保險公司呈交他們的針灸醫療費用收據，以得到經濟補償， 保險公司列舉了有關報銷收據的注意事項﹕

收據單上必須具有針灸診所或單位的印章；印章上包括診所或單位的名字， 詳細地址，電話和加拿大中醫藥針灸學會會員號。

收據單上列寫病人名字，看病日期， 所提供的系列針灸醫療服務事項，及費用價錢。

保險公司接受手寫的收據單；但是，一個收據單只能有一個醫療服務事項。 舉一個例子， 你可以手寫一個針灸治療的收據，接著另寫一個中藥收據。手寫的收據單同樣必須具有以上的規定事項。收據复件必須要合理的標誌﹕可以使用印章，或手寫在收據單上，以注明這是個收據單的复件。每一個病人必須有他自己的收據。 舉個例子， 如你治療全家人， 每個家庭成員必須有他個人的治療費用收據。 保險公司也接受發票單。但是發票單上必須蓋有“付清”的印章，同時包括生意公司的名字，和有關公司和病人的規定要求事項如上所列。
